Bin Form

Before submitting your bin request, please have your SWEK Property Assessment Number handy. You can find this number on your rates notice.

Please also have the following information ready, where applicable:

• Photos of any damaged bins.
• The bin Serial Number if you are requesting the replacement or removal of a bin. The Serial Number can be found on the outside of the bin, near the top.

If you need to submit more than one Bin Request Type, please complete a separate form for each request.
